Racking In or Out a Power Circuit Breaker is Simple isn't it? (Circuit Breakers, Safety)

NFPA 70E and CSA Z462 require employers to have an Electrical Safety Program and within it, it shall include a risk assessment procedure.  Two of the methods, substitution and engineering "safety by design” or "prevention through design," are a priority.  With them come challenges with technical equipment specification changes from traditional designs, even something as "simple" as racking circuit breakers in or out. The question is: “Have Qualified Persons been appropriately trained and understand the "substitution" or engineering "safety by design" or "prevention through design" changes from older electrical equipment they may be very familiar with? Are we increasing risk with new designs?  This presentation will review a qualitative risk-assessment process, and will focus attention on the likelihood of occurrent and human performance related to advancements in technology.  Is racking a power circuit breaker in or out simple?
